Notes:
This form must be submitted at least 7 days before each activity taking place outside the District.
A validation check is made on the information entered before the form is processed, you will be advised of any missing information. Remember, whenever an activity takes place outside the District a Home Contact must be appointed.
In addition to the information going to the District, a copy is also kept in County records.
